Practitioners wishing to refer a case can fill in the form below, or record permission in the clinical history and send this to contact@prehabvet.com.

Referrals cannot be processed without a complete medical history.  If you would prefer to discuss a case before referral, please do not hesitate to contact us. If the client e-mail address is known, please fill in this field as we can confirm receipt of their appointment and invite them to their pet’s account.

Please note we provide an out of hours contact for clients with patients under our care, to advise on any concerns directly relating to our treatment in clinic. We cannot provide emergency out of hours care, and for concerns outside their scope of referral, clients will be directed back to yourselves for further care and investigations.
